Billing preview is what the next invoice is expected to come to, not a quote from Stripe:
Stripe's own preview total,
plus per-patient money already sitting on a Stripe pending invoice item
(Stripe holds it and folds it on at finalisation — only its preview endpoint hides it),
minus whatever the subscription's own coupon will take off that pending money.
Per-patient fees still only in our ledger are listed but
never summed in, and anything ambiguous
